MEMBER TERMS & CONDITIONS
1. Medifam of California, Inc. (hereinafter referred to as Medifam) offers health care access to members through its proprietary network of medical providers and other contracted health care networks. In order to access these networks and benefits the member or member's dependents must pay the contracted reduced fees directly to the medical provider at the time of service. If a member has difficulty using the program, our customer services department is available for assistance by calling (866) 633-6680.

2. Member understands that Medifam is not an insurance company. Medifam will not issue payments or reimbursements to medical providers or members. Neither Medifam, nor its affiliates, nor any network accessed shall be liable for any payment to a provider accessed as a member of Medifam.

3. A directory of participating providers and information about the option selected is provided to member upon enrollment. The providers listed in the directory are subject to change without notice. Member is asked to call Medifam's customer service department at (866) 633-6680 for current provider information.

4. Medifam does not practice medicine and does not interfere with or participate in the provider-patient relationship. All health care decisions are between the patient and provider. The selection of a provider is the decision of the patient and is not based upon any recommendation by Medifam. Medifam's participating providers are independent contractors and Medifam is not responsible for the health care provided by any provider.

5. Minimum Enrollment Period: Because there are no eligibility requirements for enrolling in Medifam all members must participate a minimum of six (6) months. Cancellations will not be processed unless the minimum period has been met and all fees are paid to Medifam. After you have completed the minimum enrollment period you may cancel your membership by providing written notice of cancellation at least 30 days in advance. Members will be charged additional membership fees if written notice in not provided. Verbal notice does not constitute cancellation.

6. Any benefits and/or savings received by member on any of the Medifam options accessed during the time membership fees are not paid will reduce any refund amount due to member, if any.

7. Medifam reserves the right to terminate any member for failure to pay a health care provider accessed as a member of Medifam.
